Elekta sets a new standard in cancer treatment with magnetic resonance radiotherapy (MR/RT). Elekta has managed to combine two worlds – the world of modern radiation therapy and high-field MRI. These two technologies were combined to create the Elekta Unity MRI-linear accelerator, a unified platform that fully integrates MRI and radiotherapy. This revolutionary and truly uncompromising technology was developed in collaboration with seven leading cancer centers and a group of 200 scientists.

The use of modern linear accelerators cannot be a 100% effective method of tumor treatment, since visualization of tumor formations and radiotherapy are performed by separate systems. In addition, 65% of all malignant neoplasms occur in soft tissues, full visualization of which can only be obtained using MRI technologies. The new Elekta Unity system is designed to fully integrate magnetic resonance imaging with linear accelerator irradiation in real time.

MEG technology is used for both clinical and research purposes. The use of 306 magnetoencephalographic (MEG) and 64 electroencephalographic (EEG) channels provides highly accurate localization of functional areas of the cerebral cortex. The unique sensor design and the use of advanced software allow data acquisition with unprecedented detail even from the deepest brain regions.

NEXT GENERATION LINEAR ACCELERATORS

The digital MR-linear accelerator, equipped with the latest multileaf collimator, is located directly on a continuously rotating gantry. This allows the use of the most modern rotational radiation therapy methods. The linear accelerator rotates in a ring around the MRI scanner, and the radiation beam passes through the inner ring of the MRI.

HIGH-FIELD 1.5T MRI

The highly sensitive 1.5 Tesla MRI system from Philips uses technology similar to diagnostic MRI systems. This allows visualization of soft tissues with high resolution simultaneously with dose delivery during treatment. The use of a high-field MRI system provides access to direct information about the biological structure of tissues during treatment.

REAL ADAPTIVE ON-LINE PLANNING

New software functionality allows full use of MRI visualization during treatment. As a result, unique capabilities have been implemented that take into account the real position of the target during treatment, thanks to continuous MR visualization, enabling real adaptive on-line planning during patient treatment.

HDR Brachytherapy Flexitron

Flexitron is the logical choice in remote afterloading. This device provides convenience and high technological level of brachytherapy procedures. The device is fully compatible with the complete set of applicators originating from the Microselectron brachytherapy system and the Oncentra system.

Brachy, and also allows working with the full spectrum of brachytherapy implants, from the simplest to the most complex. Flexitron is a valuable long-term asset for your clinical practice.

The Flexitron family includes various configurations. For the first time, a brachytherapy device can work with both cobalt (Co-60) and iridium sources (Ir-192).

The choice of device depends on patient flow at each individual medical institution. The choice of radiation source depends primarily on logistics capabilities for source delivery and the number of patients.

For example, high patient flow and use of multi-channel procedures require systems with iridium sources. Limited source import capabilities, combined with low patient flow, allows installing a device with a cobalt source.

The choice of number of channels directly depends on pathologies and brachytherapy techniques.

10-channel systems are mainly used for treating gynecological pathologies, esophageal pathologies, bronchi, etc.

20-channel systems allow performing all standard brachytherapy procedures, including prostate, breast, head and neck treatment, etc.

40-channel systems have no application limitations, including the most advanced radiotherapy techniques.

The Flexitron® system brings new standards to radiation therapy:

  • Standard transfer tube length: no longer necessary to enter tube length values. All tubes have a standard length of 1000 mm.
  • Source stop position is reflected in position value encoding

Numerical encoding of metric source stop position values fully corresponds to its actual position: 1 = 1 mm, 400 = 400 mm

  • Zero point at applicator entry point. Simply start counting at the beginning, eliminating the need for distal tip measurements.
  • Source movement only FORWARD! Step-by-step forward source movement has been Elekta's hallmark in brachytherapy for many years.

Oncentra® Brachy Planning System

The creation of the Oncentra Brachy system is another stage in the evolution of radiation therapy planning technologies. Rich experience in creating innovative planning technologies from the first NPS solution to the famous PLATO platform, which became a kind of quality standard in brachytherapy planning, is fully embodied in the Oncentra Brachy system, equipped with the latest volume-based image planning function.

Oncentra Brachy is a comprehensive radiation therapy planning system with a user-friendly interface, equipped with the most modern dose optimization algorithms. Creating highly conformal therapy plans within an efficient workflow is now quite accessible. Oncentra Brachy provides you with the latest tools to help create the most effective brachytherapy plan.

Three-Dimensional Contouring

The Oncentra Brachy system allows you to freely navigate within the image set regardless of the acquisition method (CT, PET/CT, ultrasound, or MRI), easily scroll through images in any plane (sagittal, axial, and arbitrary), which gives complete freedom of action when drawing contours of study areas. The combination of arbitrary plane display capability with other tools, such as the smooth contour drawing tool, allows real-time three-dimensional contouring. This same free navigation function in arbitrary planes is used in catheter reconstruction mode, which makes it possible to view the entire image data set related to a specific applicator position. This easy-to-use function provides fast and very accurate reconstruction, as well as more accurate radiation dose distribution.

Reconstruction

Implant and applicator modeling.

The Oncentra Brachy system is equipped with a new implant modeling tool that allows instant reconstruction of implants with multiple catheters. The function of transferring three-dimensional geometric parameters of the applicator to image sets provides fast and accurate reconstruction. Using an additional coordinate system (ECS), you can navigate to the corresponding planar projections and move preset applicator points on them to align with the image set reference points. After placing the applicator, its position can be adjusted if necessary. These functions save significant time since reconstructed implants can be reused.

Dosimetry Optimization

Automatic dose optimization

When planning radiation therapy, it is necessary to simultaneously determine optimal target coverage, ensure uniform dose distribution, and protect organs at risk. The Oncentra Brachy system contains two inverse optimization algorithms that allow automatic calculation of optimal radiation dose distribution taking into account clinical constraints set by the user. At the same time, the user retains full control over automated processes. You can set constraints yourself, as well as combine inverse optimization with any planning and optimization method, for example, with manual source dwell time changes or graphical optimization. This allows you to calculate optimal dose distribution much faster and reduce the number of complex operations for its adjustment.

HIPO and IPSA modules include tools that provide additional optimization of radiation therapy plans.

IPSA module is equipped with a dwell time deviation constant (DTDC) function that allows creating reliable and justified radiation therapy plans by limiting significant deviations in source dwell time values.

HIPO module optimizes radiation therapy plans using the dwell time gradient restriction (DTGR) parameter, which controls significant deviations in source dwell time values in either direction, as well as needle fixation, which allows blocking part of the implant that does not require further optimization and focusing on the area where additional actions are needed, for example, inserting additional brachytherapy needles.

Advanced Collapsed Cone Engine (ACE) Dose Calculation

The Oncentra® Brachy system, equipped with the new Advanced Collapsed Cone Engine (ACE) dose calculation tool, is another step on the path of innovative medical technology development. This brachytherapy dose calculation algorithm is based on TG-186 recommendations published in 2012 by the American Association of Physicists in Medicine (AAPM) task group.

The ACE tool, fully integrated with the Oncentra Brachy system, can differentiate tissues, air, and bone tissue and take into account factors such as tissue heterogeneity, lack of backscatter in areas where there are no tissues, as well as applicator attenuation and shielding. In terms of accuracy, the ACE algorithm is comparable to the Monte Carlo method, but its use speeds up calculations and provides a more accurate picture of actual dose distribution compared to the TG-43 calculation method, which ultimately leads to more informed decisions when prescribing radiation therapy.
